Epworth will list your name in our annual report unless you use the provided checkbox to remain anonymous. When you are placing a donation in honor or memory of another person, Epworth will list that person’s name in our annual report, unless you use the provided checkbox to remain anonymous.

Privacy of Children

Epworth’s website is designed for a general audience. It is not the policy of Epworth to intentionally collect information on minors. Minors are asked to not submit any personal information on the website but rather call the Emergency Hotline at 1-800-899-5437 for assistance.
